Concerns they commonly address
Non-healing wounds
The bacteria driving non-healing wounds are invisible to the naked eye, and the practice uses imaging to detect them.
Avoidable transfers
The practice offers root-cause wound management and rehospitalization review to reduce avoidable transfers.
Services
Wound Care (2 days per week)
Provider-led wound rounds twice a week (Tuesday and Friday) with MolecuLight fluorescence imaging on every active wound.
Primary-Care Coordination
An embedded advanced-practice provider runs the day-to-day work on days the facility's PCPs are not on site, and hands decisions back to the attending physicians.
Fully Embedded (5 days per week)
Wound care and primary-care coordination combined in one program, run on separate rounding days across the week.
