48-hour appointment change policy
A fee equal to the total cost of the appointment is charged when less than 48 hours' notice is given for a change or no-show.
Forms and letters
FMLA and disability forms cost $50 with a 3-5 business day turnaround, and accommodation letters cost $10-20.
