Direct primary care memberships: $49-$179/month. Hormone Health & Weight Loss Membership: $99/month. Children up to 17 years old: $49 monthly. Adults ages 18-45: $79 monthly. Adults ages 46-64: $99 monthly. Adults 65 and over: $119 monthly.
Re-enrollment fee
Returning patients who cancelled and wish to re-enroll pay a re-enrollment fee.
Unique Abilities evaluation cost
Cost for pediatric Unique Abilities evaluations, including evaluation, diagnosis and report.
