In-Home Brain Fitness
Includes cognitive assessments, personalized in-home programming, real-time feedback, activity stimulation, reminiscent therapy, and help with coping mechanisms in the home.
Personal Assistant and Companionship Services
Supportive services for Care Management clients in Orange and Seminole counties, including companionship, light housekeeping, meal preparation assistance, errands, and transportation assistance.
Nutritional Support
Includes nutritional assessments, personalized recommendations, nutritional education, and on-going support.
Founder
Lonette Bolden-Smith, MA, BSN, RN, CMC, CDP, CADDCT
Holds a Master of Arts in Counseling and a Bachelor of Science in Nursing. Served as Social Services Director in a local nursing home for more than a decade before working in nursing, discharge planning, and community settings. Serves on the Board of Directors for the Florida Chapter of the Aging Life Care Association and is a Certified Alzheimer's Disease and Dementia Care Trainer.
