Established patients can see their HPG care team by HIPAA-secure video call, Monday through Friday during clinic hours.

CMS-approved dementia care with care navigation, respite services and a 24/7 caregiver support line.
SNFIST Program
Dedicated physician coverage is provided inside skilled nursing facilities.
Transitional care
Hospital-to-home coordination is provided before discharge, with a physician visit in the home within 7 days.
